About Department The Access and Support Service is part of the Home and Community Care Program for Younger People (HACC-PYP) and Commonwealth Home Support Program (CHSP) within St Vincents Hospital, Melbourne, which also includes nursing, allied health and a social support group. The Access and Support worker is co-located wthin the St. Vincents At Home (SVAH) nursing service, based at Fitzroy. About the Role To support HACC-PYP and CHSP eligible clients from the diverse special needs groups to access services by ensuring appropriate needs identification, assessment, and care planning based on the persons expressed goals, wishes and needs. People accessing A&S services must be seeking services and face barriers to accessing services, or staying in services, because they: * Are unsure about how to communicate their need for assistance * Have little or no knowledge about services * Are not aware of their eligibility services or lack confidence to find out if or how services could potentially help them * Think that services may not understand their needs or respect their preferences. Classifications for this position will range from Welfare Worker Grade 1 XL8 - XL14 ($52,197 - $65,811 per annum). Duties * Provide short-term, episodic support to HACC-PYP and CHSP eligible clients needing to engage with services using a Wellness and Reablement approach * Use strategies to empower eligible clients, their carers, and build their confidence in relation to access and use of services; discuss how services work; and explain and/or provide assistance to complete forms and answer questions * Actively develop and maintain links with HACC-PYP, CHSP and other service providers by working proactively and collaboratively with service providers to better meet the diverse needs of eligible individual clients. * Support the client during the care planning process; assist in developing a Wellness and Reablement focused care plan which the client/carer understands and ensure the care plan is appropriate to the clients expressed diverse needs Qualifications, Skills and Experience * Post secondary qualifications in an area of health and/or community services practice: Welfare Studies, Disability Studies, Certificate IV in Aged Care or a related discipline * Current Drivers Licence * Demonstrated high level interpersonal skills and the ability to deal sensitively with a diverse range of individuals and service providers * Demonstrated advocacy skills - seeks to understand issues and processes to be followed and provides sound advice and assistance based on knowledge and experience * Knowledge of current HACC-PYP and CHSP service provision is desirable Benefits * Salary packaging * Employee Assistance Program * Staff Health Centre * Staff Car Park Application * Please attach your resume and cover letter to your application.
