Randstad are recruiting for a Qualified Social Worker to join Tameside Council's Hospital Discharge Team. This is a full time role offering a fantastic pay rate of 30 per hour.

Responsibilities:
- You will assess individuals' needs with emphasis on early intervention and prevention and those with more complex needs.
- You will engage with the customer and carer in the process and be accountable for your own practice.
- You will produce support plans which are clearly linked to need with a clear decision on what the Department will be providing to meet positive outcomes to identify unmet needs.
- You will implement the support plan which will involve utilising approved agencies in the delivery of care and support, purchasing from the independent sector as appropriate within delegated powers.
- You will coordinate those individuals with complex needs to ensure compliance with the support plan.
- You work in partnership with other agencies including health, education and housing.
- You will monitor changing need and to re-assess and adjust the support plan and outcomes.
- You will comply with Departmental policies and procedures and quality standards.
- You will work to the standards of the Service and the corporate standards on customer care and the Citizen's charter.
- You will exercise decision making with reference to eligibility, criteria and prioritising of work.
- You will engage in regular supervision and to develop an annual training plan based on individual needs.
- You will develop the service and assist in continuous improvement programmes.
- You will maintain a recording system based on using the IAS system and ESCR which will require keyboard skills.
- You will collaborate with colleagues to ensure the most efficient delivery of services across all teams and areas.
Requirements:
- Social Work Degree or equivalent
- Social Work England registered
- Enhanced DBS
