Overview Immanuel Pathways in Southwest Iowa is seeking a Full Time Clinical Social Worker. This position comes with a competitive compensation and benefits package. The role will plan, organize and implement mental health services to Immanuel Pathways participants and families. Responsibilities include but are not limited: Complete initial assessments for psychotherapy needs, provide psychotherapy services to participants, and provide education to participants, families, caregivers as well as staff. Interventions include carrying a caseload of participants who engage in psychotherapy, lead psychotherapy and/or coping skills groups for participants, provide consultation services to PCMSWs, PLMHPs, BSWs, and practicum students, provide psychoeducation to participants and families regarding mental health issues, provide psychoeducation and support to staff members regarding mental health issues, self-care and overall coping skills, and collaborate with clinical staff and contracted psychiatry providers regarding care management. The counselor acts as an advocate for participants regarding mental health concerns and collaborates with the Interdisciplinary Team (IDT), family members and contracted providers as indicated. Responsibilities KEY RESPONSIBILITIES Key Areas Key Responsibilities and Duties of the Job 70% Participant Assistance within the center or in home
  • Performs initial intake assessments for Immanuel Pathways participants requesting or referred for psychotherapy services. Refers and collaborates with appropriate professionals for diagnosis.
  • Provides evidence based treatment for participants who have been identified with behavioral health concerns. There may be a wide range of contributing factors to current issues including but not limited to: emotional, physical, spiritual distress, major mental health disorders, grief and loss, health related concerns, family issues, interpersonal difficulties, or living environment. Makes appropriate referrals to clinic and/or psychiatry when indicated. Collaborates with clinic regarding behavioral health concerns.
  • Performs home visits to participants who are unable to come to center due to severity of mental health concerns and/or noncompliance. Completes in home psychotherapy if indicated.
  • Collaborates with Interdisciplinary Team (IDT) to complete plan of care for participants who are currently on caseload or have been referred for psychotherapy services. Maintains regular attendance at, and participates in team meetings; communicates participant changes as appropriate, collaborates on plan of care decisions and coordination for twenty-four (24) hour care delivery.
  • Provides ongoing support, counsel and psychoeducation to participants and family regarding a variety of issues but not limited to: the aging process, dementia, grief and loss, end of life, disease processes, difficult family dynamics, changing life roles, development and course of mental health disorders.
  • Attends and actively participates in a variety of organizational meetings related to participant care, including but not limited to: Morning meeting, care plan meetings and report outs for participants on caseload. Attends psychiatry appointments with participants (with their permission or request) as indicated.
  • In the event of termination of the PACE organization, the clinical social worker will make appropriate community referrals for psychotherapy services, so treatment may be continued if needed.
  • Completes and ensures completion of documentation of clinical service, in participant's medical records including psychotherapy session, home visits, hospital visits, or conversations with contracted providers according to Pathways Documentation requirements.
  • Performs initial diagnostic assessments for Immanuel Pathways participants requesting or referred for psychotherapy services within scope of practice (LISCW only).
15% Supervision, Consultation and Staff Education
  • Participate in consultation groups or sessions with other members of Behavioral/Mental health department in both Iowa and Nebraska. May provide supervision to PCMSWs, PLMHPs, BSWs, and practicum students through individual or group meetings (depending upon licensure requirements). Attend research/educational group for mental health providers to ensure evidenced based practices are being utilized with participants.
  • Provides psychoeducation and support to staff members regarding mental health issues, self-care and overall coping skills. Acts as a resource to other team members and participant center staff regarding topics such as dementia, difficult behaviors, and difficult personalities.
10% Compliance
  • Acts within the scope of his or her authority to practice.
  • Implements Exposure Control Plan.
  • Complies with Emergency Preparedness Plan.
  • Protects privacy and maintains confidentiality of all company procedures, results and information about employees, participants and families.
  • Appropriately labels Behavioral Health notes to ensure confidentiality requirements specific to psychotherapy notes.
  • Follow stated guidelines specific to privacy and Behavioral Health and Substance Abuse information.
  • Participates in and supports Quality Improvement Initiatives.
  • Completes Security Review process system access in the respective state in which he/she is employed.
  • Maintains up to date knowledge of system/process and communicates to IDT as applicable.
5% Other
  • Performs other duties as assigned or requested.
  • Masters Degree from an accredited college in a human service program which provided preparation for professional counseling.
  • Licensed as a mental health practitioner in the State of Iowa.
  • Two (2) years of experience working on a multidisciplinary team in a hospital, clinic, nursing home or community based setting is preferred.
  • One (1) year of experience working with the frail or elderly population required, or completion of job specific training related to working with the elderly population must be completed within the first six months of hire.
  • One (1) year of leadership or management experience, preferable in geriatric care setting preferred.
Other Requirements
  • Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
  • Basic Life Support (BLS) Skills (i.e. Health Care Level Cardiopulmonary resuscitation (CPR); Automated External Defibrillator (AED); First Aid (FA).
  • Must have a valid driver's license, proof of insurance or have means of transportation.
KSA Knowledge Skills and Abilities -
  • Knowledge of clinical social work, biopsychosocial assessment, psychotherapy, and evidence based treatments for Major Mental Health Disorders as well as other Adjustment Disorders or life events.
  • Knowledge of personal theoretical orientation and approach to treatment. Able to adapt to theoretical orientation based on client need.
  • Skilled in providing effective individual and group psychotherapy to participants who have a variety of mental health concerns ranging from grief and loss to Major Mental Health Disorders.
  • Skilled in verbal and written communication, including speaking to groups of people.
  • Skilled in designing and implementing psychoeducation presentations to various groups (e.g. participants, family, staff, contracted providers).
  • Ability to work with frail/chronically ill elderly people.
  • Ability to maintain accurate records and to prepare clear and concise reports, correspondence and other written materials.
  • Ability to communicate clearly and effectively.
  • Ability to chart via Electronic health Records.
  • Proven experience and basic computer proficiency (internet, email, Microsoft Office).
