The therapist functions as a core member of Metro's Integrated Care Services (ICS) Team, providing mental health care in collaboration with the patient's primary care provider, a psychiatrist, and the program's Care Coordinator. The therapist is responsible for providing individual and group therapy for patients' overall mental wellness, including but not limited to managing chronic physical and mental illness, depression, anxiety, trauma/PTSD, grief, bipolar disorder, schizophrenia, personality disorders. The therapist will provide knowledgeable and affirming care to a diverse urban under-served population, including persons of color, immigrants, and LGBTQI+ identified individuals. The therapist will assist the Care Coordinator in coordinating and supporting mental health care within the primary care side of the clinic and coordinating resources and referrals to services outside the clinic, if indicated.



Requirements:

ESSENTIAL FUNCTIONS:

  • ASSIST WITH THE SCREENING OF PATIENTS
  • Providing consultation to medical staff regarding the screening of depression, anxiety, and unhealthy substance use. The PHQ-4, PHQ-9, GAD, AUDIT and DAST are tools that will be used in the screening and before ongoing encounters to measure patient needs progress and lack of progress.


  • ENGAGEMENT OF PATENTS
  • Providing motivational interviewing, outreach phone calls, phone reminders of appointments and phone calls to follow up on progress.


  • ASSISTING PATIENTS TO ACHIEVE GOALS
  • Engaging the client in conducting assessments, determining goals, developing a treatment service plan and providing various therapeutic and educational methods to achieve desired outcomes.


  • WORKING WITH CLIENTS IN A BRIEF PROBLEM-SOLVING APPROACH
  • Using time sensitive modalities and methods of therapy, routinely evaluating with the client progress towards achievement of goals and terminating within allotted time frames.


  • CONSULTING WITH MEDICAL COLLEAGUES AND BH RESOURCES
  • Providing consultation to the interdisciplinary team including primary care physicians, registered nurses, medical assistants, front desk staff, pharmacists, BH case manager and community education specialists to improve the medical and behavioral health level of functioning.


  • HANDLING A RANGE OF PATIENT SITUATIONS
  • Developing the capacity to employ diverse brief treatment modalities relevant to individual, families, children, and couples. Providing crisis intervention and facilitating emergency procedures. Assessing effects of social, cultural and community factors on consumers' lives, identifying and networking with internal and external resources, advocating for client, referring of consumer and collaborating around consumer concerns to facilitate smooth transitions.


  • MANAGING WORKLOAD REQUIREMENTS
  • Maintaining the productivity expectation of the program for service; scheduling flexibly to serve both new and existing consumers.



  • DEVELOPING RELATIONSHIPS WITH THIRD PARTY PAYERS
  • Knowing and adhering to current third-party payer criteria, communicating with authorizing personnel, completing required documentation, advocating for the consumer's needs and promoting program services.


  • MAINTAINING AND CONTRIBUTING TO PROFESSIONAL COMPETENCY
  • Utilizing professional development and training opportunities. Developing and conducting staff training in area(s) of expertise.
