A rare and exciting opportunity has arisen for a mental health Band 3 support worker to join the Older People's Crisis Resolution Home treatment team based in Peterborough. The successful candidate will be working within the Dementia Intensive Support Pathway within CRHTT-OP working across Peterborough, Fenland and Hunts.
The CRHTT-OP/DIST key objectives are to focus on home treatment for patients and their carers living with dementia. This includes the management of risk, prevention of hospital admissions and working with carers to prevent carer burnout and care placement breakdown.
The successful candidate will bring not only experience of supporting people living with dementia but an ability to think 'outside of the box', be imaginative, innovative and highly motivated to make a difference. You will need to hold a full UK driving licence and have use of a vehicle.
