Summary The Registered Nurse Outpatient Clinic Patient Aligned Care Team (PACT) maintains the health and manages the care for patients with a variety of medical problems including hypertension, diabetes, arthritis, cancer, congestive heart failure and other chronic diseases. The Registered Nurse Outpatient Mental Health utilizes the nursing process for systematic data gathering, assessment, appropriate nursing judgment, and evaluation of human responses to actual or potential health problems. Responsibilities The Registered Nurse, Greenville Outpatient Clinic Patient Aligned Care Team (PACT) will serve as a primary care nurse in support of the physician or advanced practice provider. Duties will include, but not be limited to, patient intake, documentation, point of care testing and health screening. The RN will assess, triage, take histories and document conditions and concerns for Veterans presenting to the clinic and via the telephone. The RN is responsible for promoting an environment that supports caring and courteous service, safe patient care, and positive working relationships. The clinical nurse will coordinate care through the health continuum using health education, health coaching, and health promotion. The Registered Nurse (Outpatient Mental Health) utilizes the nursing process for systematic data gathering, assessment, appropriate nursing judgment, and evaluation of human responses to actual or potential health problems. Provides services for case findings, health teaching, health counseling; provision of care supportive to or restorative of life and well-being; and executing medical regimens including administering medications and treatments prescribed by a licensed or otherwise legally authorized provider. The nurse's primary commitment is to the patient, whether an individual, family, group, or community (ANA code of Ethics for Nurses). The Outpatient Mental Health Registered Nurse performs the following duties: Develops and maintains therapeutic relationships with all patients. Assesses the physical and mental health needs of assigned patients. Completes suicide risk screenings, documents screening in Computerized Patient Record System (CPRS) and implements nursing interventions based on Suicide Risk Assessment (SRA). Promotes mental health by implementing nursing interventions for behavioral problems, psychiatric disorders and co-occurring conditions to promote health and safety, symptom management. Provides age-appropriate care in a culturally and ethically sensitive manner. Responsible for maintaining Preventative and Management Disruptive Behavior (PMDB), Basic Life Support (BLS), and other required trainings. Manages psychiatric emergencies and intervenes with therapeutic principles of PMDB when clinically indicated. Provides structure and maintains a safe therapeutic milieu & recovery-oriented environment. Collaborates with treatment team to develop and revise a patient-centered plan of care. Documents nursing plan of care in Mental health Suite appropriately within required time-frames. Implements nursing interventions to assist Veterans in their recovery Demonstrates knowledge of psychotropic medications and other prescribed medications. Administers and monitors for effectiveness and reactions to psychotropic mediations and other prescribed medications. Work Schedule: Monday - Friday, 8:00 a.m. - 4:30 p.m. Tour subject to changes based on the needs of the agency. Financial Disclosure Report: Not required. Telework: Not Authorized. Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Preferred Experience: Minimum of three years of successful clinical practice. Prior recent experience in ambulatory care/primary care nursing, including care of patients with mental health, female Veterans, and health education needs. Previous experience in the care manager role and coordinating care/care delivery. Previous experience with telephone advice nursing. Demonstrated proficiency in using the nursing process in providing care for clients with complex care needs. Previous knowledge/experience with the patient centered medical home practice model. Demonstrated ability in the areas of interpersonal relationships, problem solving/critical thinking skills and conflict resolution. Preferred Level of Education: BSN Preferred Certifications in: Ambulatory Care Case Management Medical - Surgical Telephone Nursing Practice Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.
