RESPONSIBILITIES:The Nurse role involves program planning and development, community-based assessment and treatment of clients, and crisis management. The Nurse will perform a wide variety of tasks related to the care, support, education, socialization, habilitation, recovery/healing, safety, security, and support of adults diagnosed with mental illness and substance use disorders who are transitioning to various levels of independent housing settings. The Nurse is responsible for the diagnosis and treatment of human responses to actual or potential health problems through such services as case finding, health teaching and counseling. The Nurse will provide interventions supportive to or restorative of life and well-being; and execute medical regimens prescribed by a physician, PNP or another licensed health care provider. Duties of the Nurse focus on assessing client health problems reflective of holistic needs, developing and implementing nursing care plans, and completing health assessments. The Nurse will consult and coordinate with healthcare team members to assess, plan, implement, or evaluate care plans. The Nurse will advise clients on health maintenance and disease prevention including effects of diet and physical activity and provide case management; provide health education and connection to primary care to address physical health issues that impact a clients ability to successfully live in the community; and provide health education for patients and their families for specific medical needs, preventative care, and how to access and utilize healthcare services in the community. Based on a comprehensive review of the client's overall condition, the nurse will continually review and evaluate plans of care and nursing interventions to ensure that such interventions are consistent with the client's comprehensive treatment plan. The Nurse is also responsible for providing direct treatment and support services to clients with psychiatric diagnoses, co-occurring substance use, chronic physical health problems and complex social needs. Service provision takes place at inpatient settings, in the clients home environment, community settings, provider offices and additional locations as needed. The Nurse is responsible for high quality care coordination services that will assist individuals in their recovery journey, keep clients well in their home and community and increase positive health outcomes.The Nurse is required to utilize technology and various web-based platforms for documenting progress notes and daily work activities. Ability to use equipment such as iPhones, tablets, Surface Pros and easily navigate various technology platforms and reporting systems is a requirement.SPECIFICATIONS FOR EDUCATION/LICENSES/CERTIFICATIONSLicense and current registration to practice as a Registered Nurse (RN) in New York StateValid NYS Driver License required. 3 years of current driving experience with no more than one moving violation in the previous 24 months (2 years), and no driving while intoxicated convictions required. SPECIFICATIONS FOR EXPERIENCE AND TRAININGOne-year post RN licensure clinical nursing experience working with individuals with behavioral health needs preferred.Knowledge of professional nursing concepts, principles and practices.Knowledge of concepts and principles from the biological sciences that are supportive of nursing practiceKnowledge of concepts and principles of health promotion and disease prevention.Knowledge of abnormal signs and symptoms relating to physical and mental illness, as well as indicators of wellness.Knowledge of medications and drugs, common dosages, their psychological, physical and physiological effects and possible adverse reactions.Knowledge of emergency treatments and interventions.Knowledge of cultural, ethnic, customs, beliefs, and linguistic needs of clients relating to cultural competence.Ability to recognize changes in signs and symptoms that warrant the immediate action of a physician.Ability to develop, evaluate, implement and modify a plan of nursing intervention to meet the needs of individual clients.Ability to secure the cooperation of and work effectively with others.Ability to prepare accurate and timely reports.Bilingual Spanish speaking preferred.Excellent oral, writing and communication skills.Ability to teach skills to individuals and groups.Free from present litigation that could result in loss of licensure.Capacity to learn computer skills.Ability to perform skillfully, communicates clearly, discuss assessments with clients.
