Adults, Housing and CommunitiesThe Adult Social Care Team forms part of Southampton City Council s Adults Housing andCommunities Directorate.The service is responsible for triaging, signposting and delivering a range of support to adults whoare deemed to have a range of care and support needs, their families and carers-both informal andformal. The broad areas of focus are:- Strong partnership working with early help services to prevent vulnerable adults from needingmore specialist help, building resilience in families and coordinating support To lead coordinated offers of support to adults who are deemed to have care and supportneeds and are unable to protect themselves or others Strong leadership and partnership with our other organisations to improve the outcomes foradults with care and support needs Assessing adults with care and support needs or a disability and applying Care Act eligibilityusing a strengths based approach Monitoring services, that will look for continual improvement that will provide care and supportfor our most vulnerable clients Ensuring that services are compliant with statutory guidance and policy and that we arecontinually improving through work force development, performance monitoring, independentreview and case auditOur aim is to promote resilience, provide the right level of support at the right time and to enableadults with care and support needs to achieve their potential and to promote a positive outcome forthem and their carers. We are working to reduce demand, increase effectiveness and transform howwe deliver services to ensure that adults with care and support needs receive the best information,advice and services to enable them to live independently and with choice within an affordable modelof workWe are looking for a dedicated Mental Health Social Worker (Band 6 equivalent) to cover as a Care Co-ordinator in our Central Community Mental Health Team (C-CMHT) working in the heart of the city. You will be covering for a post holder who has taken up a secondment opportunity as a senior practitioner elsewhere in the service and you will be required for a period up to 6 months (reviewed at 3 month intervals) to provide essential backfill. You will be a skilled and experienced Social Worker who has undertaken the role of Care Co-ordinator before and understands the requirements of the adults and families living with severe and enduring mental health needs. You will also be able to discharge the functions of the Care Act 2014 frameworks, as we are in a section 75 NHS Act 2006 agreement within Southampton. You will be skilled in acting as a safeguarding practitioner and able to undertake detailed CMHT generic assessments and Care Act led assessments. You will also be asked to undertake Shared Care (duty) function within the team. You will have the supported of a dedicated team of Care Workers who can assist with the functions under the Care Act and a skilled workforce of Support Time Recovery Workers. You will thrive on working in a busy and fast paced team and within the Central CMHT, you will be supported by three Team Leaders - 2 nurse team leaders and 1 senior social work team leader and a Team Manager.
