Job Summary

Using interviewing, organizational and networking skills, gathers clinically relevant information and provides psychosocial assessment to assist the healthcare team in pre-surgical clearances and developing a treatment plan for the bariatric patient. Actively collaborates with healthcare team to meet patient care outcome management and system improvement goals. Uses planning, prioritizing and organizational skills to achieve utilization and quality goals.

Minimum Qualifications

Minimum Education: Bachelor's Degree in Social Work and Master s Degree in Social Work from accredited programs from an Accredited Program
Minimum Years of Experience: None.
Substitutable Education & Experience (Optional): None.
Required Certifications/Licensure: Licensure as an Independent Social Worker-Clinical Practice in the state of South Carolina
Required Training: Experience with psychological assessments (i.e., personality assessments, depression assessments) and evaluation

  • Bariatric Assessment/Program: Perform a variety of duties to support and in support of Bariatric Services.
  • Conduct preoperative psychological evaluations and provide recommendations to the clinical team on the appropriateness of surgery from a mental health perspective for surgical candidates.
  • Coordinate, lead and facilitate monthly pre-operative behavioral workshops.
  • Counsel, coach and educate patients and family on lifestyle changes and setting goals pre-operatively and post-operatively.
  • Provide support to patients and education to team members and staff.
Duties & Responsibilities
  • Lead evening post-operative support groups with other bariatric team members.
  • Contribute to research, quality initiatives and the development of curriculum and services for the behavioral aspects of surgical and non-surgical weight loss programs.
  • Documents assessment information in the patient s medical record (EMR).
  • Conveys and receives information efficiently to and from patients, families, other members of the health care team and outside facilities and agencies.
  • Provides education/consultation to the patient/family and healthcare team.
  • Determines comprehension and need for repetition and revision of methods of teaching.
  • Uses appropriate channels for reporting progress and/or concerns.
  • Consults with attending physician to coordinate care plans for the patient.
  • Identifies opportunities for improvement and coordinates/participates in the development and implementation of action plans to make improvements.
  • Contributes to patient outcome measurement.
  • Participates in Bariatric Team meetings while acting as a patient advocate.
  • Bariatric program development to assist with maintaining program accreditations.
  • Participates on teams designed to improve systems based on analysis of data and evaluates outcomes.
  • Participates in bariatric activities and in interdisciplinary patient care conferences
  • Performs all other duties as assigned by authorized personnel or as required in an emergency (e.g., fire or disaster).
